If a registered veterinary premises is closing, the Certificate of Suitability (COS) Holder is required to
notify the Council by submitting a COS Revocation form, available here. The completed form can be
returned by post to the VCI office or a scanned copy by email to pas@vci.ie.
The COS Holder is also required to update their employment details by submitting an updated PAS
Declaration form which is available through the following link – Update your PAS Declaration
Once the COS Revocation Request is submitted, it is first considered by the Practice Premises
Committee (PPC), followed by the Council at its next possible meeting. For a list of meeting dates

Please note that once a COS is revoked, the practice of veterinary medicine at and/or from this
premises is no longer permitted under section 106 (1)(a) of the Veterinary Practice Act 2005, as
amended, and the Department of Agriculture, Food and the Marine will be notified of such.
Upon the revocation of a COS, before its expiration, where the practice of veterinary medicine
at/from the premises will cease, a COS Holder may apply to the Council for a partial refund of the
COS fee not utilised up to the date of revocation. To avail of the refund, please return a completed
beneficiary form beneficiary form when submitting your COS Revocation Request.
